Bankruptcy Counseling Information

  1. I understand the agency will provide a comprehensive consumer education program that has been approved by the Department of Justice. A qualified educational presenter will conduct the education program required by the Executive Office of the United States Trustees.
  2. I understand that if I am dissatisfied, I can utilize the Complaint resolution Process.
  3. I understand that the purpose of the educational services offered is to help me better develop and understand financial skills.
  4. I understand that upon the completion of the course I will be granted a certificate of completion.
  5. I agree to hold the employees of Consumer Credit Counseling Service of Madison (a division of Family Service, Inc.) harmless from any claim, suit, action or demand of my creditors, myself or any other person arising out of this agreement or resulting from advice, education or counseling.
  6. Nothing herein shall apply to actions or claims under the provision of the United States Bankruptcy Code, 11 U.S.C. Sec. 101 et seq.
  7. No student will be turned away for inability to pay. Services are provided without regard to ability to pay the fee.
  8. I understand that Family Service Inc. does not pay or receive fees or other consideration for the referral of debtor students to Family Service Inc.
  9. The course will be held once a month at the Family Service, Inc. location 128 East Olin Ave, Madison WI 53713. All courses will be held from 9am to 11am on the third Saturday of each month.
  10. Course instructors are NFCC certified consumer credit counselors.

There is a $50 per person fee for pre-bankruptcy credit counseling and $25 per person fee for Personal Financial Management Instructional Course.
